Reprenant des éléments du film E.T. l’extra-terrestre dont il est l’adaptation, le jeu ne reprend pas pour autant la trame générale du scénario. Le joueur man?uvre E.T. à travers différents écrans à la recherche des trois pièces de l’émetteur. Les pièces sont réparties aléatoirement autour de la zone d’atterrissage d’E.T. dans des fossés appelés « puits ». La zone de jeu est composée de six écrans principaux. Le joueur doit éviter le contact avec certains ennemis.
